So basically I've been wondering, is it a harem if the girl has split personalities and the guy is dating all of them? What about if the guy has split personalities and each personality is dating a different girl? What if you have multiple bodies where you share thoughts and feelings with all of them but each body is stuck in a different dimension/world, would that make it a harem even if each body was true to one girl? So many questions, so few answers.
Split personalities don't count i.e. Launch from DB. Different bodies in different worlds depends on how they share consciousness but at best it's polygamy and not a harem since there's no 'harem dynamics' or 'interactions'. One guy with multiple personalities dating a girl each is a harem, because we identify with MC, all parts of him (some more than others) and these girls will also probably interact.
One premise I've always wondered if what if the guy was dating a monster girl, and the girl was a two headed ogre and each head had a unique personality and will of its own. Would that be a harem, or would it not count because the heads share the same body?

Because for an ogre to have two heads is a natural trait found in their species, like a goblin having green skin, or a cyclops having one eye. Also since it is a fictional thing it is easier to discuss it, and fantasize about it. Conjoined twins on the other hand is birth defect, and like you said a real world phenomenon. Joking around about whether dating a conjoined twin would count as a harem just feels disrespectful, and distasteful.
You know how 'twins' have a 'mental connection' in certain stories? In that story they were mentally connected so far that they considered themselves as a single entity. Like they spoke at the same time, they shared touch or whatever. Not like it's a triple siamese twin, lol. This brings up the question, if you do a siamese twin is it a threesome? I've actually seen an article where one of them got married, but the body is shared. Another question, if two people are sharing their feelings and you know it but have sex with one while the other doesn't want to but is forced to feel it, is it rape?
IMO a harem is one man one body + more than 2 women in individual bodies IMO split personalities are classified in the name, "split" as in they are split from the whole and count as one all together.
I don't consider any of the above a harem, and will just answer with the nornal meaning of the word, which is one person with multiple spouses/concubines. Edit: I would like to add that just dating several people wouldn't make it a harem as long as they don't know about each other, imo. So the girls who think they are the only girl for him are not in a harem. And I would really not say that split personalities apply, since in real life I don't think it would be applicable. And that about multiple heads one boso sounds like a scenario where the heads would, out of convenience have to agree on things like who to date so I wouldn't count it.

None of the above i think, here's why: A harem is originally a part of the house/ castle/ palace that is dedicated to women where the females of the family reside. This includes wives, concubines, daughters & mothers (step or otherwise) aunts maids etc... A harem usually belongs to a single patriarch and is normally forbidden to enter for males outside the family, which makes sense considering its derived from the word haram that translates into forbidden / sin.
