You suddenly appear in a brightly lit white room sitting in a chair with two big red buttons marked A and B in front of you. You are then told by a cold ethereal voice that if you push button B then a random 90 year old on earth will die as a result, and if you push button A then a random newborn baby on earth will die as a result. Next, you're told if you don't press one of the button within ten seconds then both the 90 year old, and the new born baby will die. What do you choose to do, and why?

I'd push both buttons! Then apologize cause I kind of pressed them both before the ethereal voice gave any explanation... I even did the whole button jam thing~

I mean I'm sure they'd understand, there were buttons begging to be pressed!

Question, is it possible to kill both if I press the two buttons?

Well, if it's not possible to kill both by pressing the two buttons, I'd press both and leave it up to date (irresponsible, I know)
But if it's possible, I'll press the 90 year old to die. I can't justify the killing, but that's what I'd pick
For now, I'll leave my choice as pressing both...

Button B to kill 90-year old since they have fewer years remaining to live. At most they would live for another 10 years, and if they are 90 then their friends and family should already be prepared for his death. Newborn baby could become that 90 year-old grandpa in 90 years, and its more tragic for a newborn baby to die then a 90-year old who has already experienced all the happiest moments of his life. Also, killing the 90-year old could be a life lesson and hold a lot of meaning and teach people how to deal with the reality of death and aging. No meaning if a newborn baby just dies, the mother and father aren't exactly gonna learn anything meaningful. Baby has more potential value.

How do they die? Instantly, or painfully?

Let me tell you a secret.

There are no buttons to kill people.
If that thing wants to kill them, it will, if it does not, it won't, simple as that. The buttons are just there to shift responsibility.

Regardless of the fact that I was 'given' a choice, and I am responsible (or feel responsible) for their fates, I'd rather not pick at all.

    Like good 'ol Geralt said
"Evil Is Evil. Lesser, Greater, Middling... Makes No Difference. The Degree Is Arbitrary. The Definition's Blurred. If I'm To Choose Between One Evil And Another... I'd Rather Not Choose At All."
    - I'm not sure if I'd never really do an evil choice when forced because I'm human and selfish, but I will always pick the choice that is achievable and the lightest on my conscience- better if it is righteous.

I'd rather stay my ground than be the one who caused the death, nor would I easily follow the prick who specifically put me in that room out of billions of people.
The fact that what I see is a only a button which only need to press to get it done, instead of seeing and meeting these people and their demise makes it easier to detach from the deaths and feel less guilt.

Inaction is also a choice, so is rebelion, so is taking a dump and covering the wall in shit. Say that you won't play by unfair rules means nothing if you can't flip the board
