Manga? Age gap? Koi Kaze, Mizu to Gin and Mizu no Iro Gin no Tsuki (Not entirely, but continues the story of Mori and Hoshi from Mizu to Gin). These are all from the same author/artist.

Also, I must warn you that these mangas have nudity/sex scenes in them. Koi Kaze has incest.

There's also Natsu no Zenjitsu and Kannou Sensei. All from the same author, but with these two, the ages doesn't match your criteria, but it does have an age gap (about 20 years with Kannou Sensei, 5 years with Natsu no Zenjitsu).

If you got interested with Mizo to Gin, Natsu no Zenjitsu is a prequel to that. Basically the timeline goes like this:
Natsu no Zenjitu > Mizu to Gin > Mizu no Iro Gin no Tsuki

There's also Watashi no Shounen, although I don't know how the story will play out since there's only so much chapters that have been translated. There's a big age gap here too, but this doesn't match your criteria as the male lead is 12 y/o(you said teen) with a 30 y/o woman. I'm just putting it here in case it matches your tastes.
