Please recommend a novel where the MC is retarded or mentally handicapped, not acting like one, but actually is. A good exame would be the early chapters of The Silly Alchemist where Ye Lang was still a stupid but good boi. To make things clear, I'm talking about real life mental deficiencies, or something close to that. Where one could be slow witted, dull, etc.

Just pick one of those isekai japanese story ....... despite being an average highschool student which would meant he had decent amount of IQ..... the mc often did one of the most idiotic and fatal mistake, and repeat it again and again........ they shouldn't be a retard, but the story showed us how much of a retard they truly are..... not all of em..... but many are.....

I'm pretty sure that almost every MC would qualify as mentally retarded. The only reason any of them survive is due to plot armor. Without it most of them would be dead due to their utter retardation. And it goes beyond just romance. But I guess if you want an MC where the author is not trying to portray them as normal mentally. Hm... for female MCs there's The Otherworldly Adventures of a Super Naive Girl and I Said Make My Abilities Average! For male MCs... most of them qualify. Particularly the ones that the author try to portray as geniuses.
