    Evading a truck but accidentally slip into a tree hole and break his neck... The sister try to save mc also slipped by banana peel and got hit by the truck.... Both reincarnated as twin. Discontinued novel however..

    Death by blood loss after single handedly descimating an entire city full of strong yakuza/gangs and reincarnating into a world without an op ability but everyone else in that city was, however still succeeding to kill everyone of them in an epic war, and returns home where he is hailed as a hero, but just as he parades around the streets before accepting the holy sword from the king to slay the demon lord, he slips on a banana peel, gets gangrene and dies in a humiliating way in front of everyone he knows and loves
