Russia once trained military dogs to carry bombs under tanks to blow them up. The problem is they used Russian tanks to train them. So when they released them in battle, the dogs promptly went under the russian tanks and blew them up.

I may as well provide a fact myself: The William D. Porter (DD-579), has experienced many an unfortunate event, like nearly sinking an allied battleship that had the US president of the time on board. There's a video about it by Potential History on YT.
A Norwegian research roket almost started WW3 during the cold war. If i remember correctly it was couse of a message delay in the SSR beurocraty and a slight deviation in the rocets course. The reason for WW3 not starting was couse someone high up in the ranks did not want to start a war. I rememberd this while watching a military tattoo. My phone lost power while writing this so i hadd to write everything once more.
