    I've come across that phrase multiples times across various novels.
    eg.He gave a smile that wasn't a smile, She smiled a smile that really wasn't a smile etc
    I did a bit of searching too but all that came up was various dental problems:blobsweat_2:

    It's an insincere smile.
    Smiling is a sign that the person is happy, but in this case the person is physically smiling but they aren't actually happy.

    Basically a wry smile
    their mouth is curved into a smile but their eyes or the rest of their face is more of concern or just plain tiredness
